Incident Report Form

It's important to speak up if you witness or experience a workplace or food incident. Whether it's a safety violation, harassment, or a food quality issue, reporting it can help protect yourself and others from harm and improve overall workplace or food safety.

Please read this document to learn when we report an incident.

(Note: All incident records will be stored for 7 years from date created) 

Please report within 24 hours for the following events:

• Death of a person with a disability
• Serious injury of a person with a disability
• Abuse or neglect of a person with a disability
• Unlawful sexual or physical contact with, or assault of, a person with a disability
• Sexual misconduct committed against, or in the presence of, a person with a disability, including grooming of the person for sexual activity

And within 5 days for the use of restrictive practice in relation to a person with a disability if the use is not in accordance with a required state or territory authorisation and/or not in accordance with a behaviour support plan.

The Commissioner will be notified of ‘Reportable Incidents’ of the following information within 24 hours:
